Eyesight is precious. Every day our eyes provide us with pictures of beauty, wonder, emotion and surprise.

If you've captured any of those moments in a photograph, we encourage you to enter it in Hart Eye Center's "Your Life in Focus" Photo Contest.

Beautiful photos. Heartwarming photos. Interesting photos. Surprising photos. Scenic photos. "Wow" photos. In short, enter your best photos that capture "Your Life in Focus."

A special Hart to Heart extra: For each photograph entered in the first 30 days of the contest (March 15–April 16), Hart Eye Center will donate $10 to the American Heart Association (up to $2,500).

Surgery will be performed by Dr. William B. Hart, and includes one year of follow-up care. You must be eighteen years of age or older in order to receive LASIK surgery. Procedure must be performed within six months of notification.

LASIK is not medically appropriate for every person. If the Grand Prize Winner is not an appropriate candidate for LASIK, as determined by Dr. William Hart, the winner may choose to do one of the following:

1. Transfer the prize to a member of their immediate family. Immediate family members include parents, step-parents, children, step-children, siblings, step-siblings, or spouses.
2. Choose to receive the equivalent dollar amount of LASIK surgery cost ($3000) as credit to be used for other services at either Hart Eye Center or Lakeside Optical. The full amount must be used within one year from date of contest closing.
3. Choose to donate the surgery to someone who would otherwise not be able to afford the procedure. Hart Eye Center will be responsible for choosing the recipient.

Winner is responsible for all federal, state and local taxes and costs and expenses associated with prize receipt and/or use not specified herein as being awarded. LASIK recipient can expect to pay for normal incidental costs associated with treatment, such as eye drops and antibiotics. Such costs do not typically exceed $100.00.

All contestants will be entered into a drawing to win a Canon PowerShot camera. One drawing entry will be awarded for each photograph submitted. The drawing will take place on July 1, 2010, and Hart Eye Center will notify the winner by e-mail or phone.
